Gerilim Tipi Basagrilarinda Öfke Düzeyi, Öfke Ifade Tarzi ve Duygu Durum Profilinin Degerlendirilmesi/Evaluation of Anger, Anger Expression Style and Mood Profile in Tension-Type Headache

ÖZET. Amaç: Bu çalismada Uluslararasi BasaGrisi TopluluGu (UBT) 2004 sinisamasi kriterlerine göre sik epizodik gerilim tipi basaGrisi (GTBA) ve kronik GTBA tanisi konan hastalar ve kontrol grubu arasinda öfke, öfke ifade tarzlari ve duygudurum profillerinin karsilastirilmasi amaçlanmistir. Yöntemler: Çalismaya UBT 2004 kriterlerine göre sik epizodik (n=28) ve kronik GTBA (n=42) tanisi konan hastalar ve kontrol grubu (n=50) olarak da gönüllüler alindi. Katilimcilara Sürekli Öfke-Öfke Tarz ÖlçeGi (SÖÖTÖ) ve Duygudurum Profili ÖlçeGi (DDPÖ) uygulandi. Katilimcilarin alt ölçeklerden aldiklari puanlar ayri ayri hesaplandi. Üç grubun yas, eGitim düzeyi ve SÖÖTÖ ve DDPÖ alt ölçek puanlari arasinda farklilik olup olmadiGi tek yönlü varyans analizi (ANOVA) ile arastirildi. Post hoc deGerlendirme Tukey testi ile yapildi. Kategorik deGiskenler karsilastirilmasinda kikare testi kullanildi. p≤0.05 istatistiksel anlamlilik kabul edildi. Bulgular: Hasta gruplarinin SÖÖTÖ'de sürekli öfke, öfke-iç ve öfke-dis puan ortalamalari arasinda anlamli fark yoktu. Ancak her iki grupta sürekli öfke ve öfke-iç puanlari kontrol grubuna göre anlamli derecede yüksekti (p<0.001), öfke-dis puanlari ise kontrol grubuna göre anlamli derecede düsüktü (p<0.001). Duygudurum profili deGerlendirilmesinde; saskinlik, yorgunluk, anksiyete, depresyon ve öfke alt ölçeklerinden alinan puanlar her iki hasta grubu arasinda farkli deGilken kontrollere göre istatistiksel olarak anlamli derecede yüksekti (sirasiyla p=0.014, p<0.001, p=0.011, p<0.001 ve p=0.003). Sonuç: Bu bulgular GTBA hastalarinda psikolojik ya da bedensel etkenlere baGli olarak artmis olan öfkenin disa yansitilamadiGini, bu nedenle içe dönük öfkenin artmis olduGunu, buna baGli olarak da somatizasyon araciliGiyla basaGrisi düzeyinin artabileceGini düsündürmektedir. Objective: In this study, we aimed to compare anger, anger expression style and mood profiles between patients with frequent episodic tension-type headache (TTH) and chronic TTH, based on the International Headache Society (IHS) 2004 classification criteria, and control subjects. Methods: Patients diagnosed with frequent episodic (n=28) and chronic TTH (n 42) according to the criteria of IHS 2004 and volunteers as control group (n=50) were enrolled in the study. The participants completed the State Trait Anger Scale (STAS) and the Profile of Mood States (POMS). Subscale scores of participants were calculated separately. Age, educational level, and subscale scores of the STAS and the POMS were analyzed to observe whether differences exist between the three groups with one-way analysis of variance (ANOVA). Post hoc evaluation was made with Tukey's test. Categorical variables were compared using the chi-square test. p≤ 0.05 was considered statistically significant. Results: There was no significant difference between the patient groups with respect to mean scores of trait-anger, anger in and anger out of the STAS. On the other hand, in both patient groups, trait-anger and anger in scores were significantly higher than those in the control group (p<0.001), while anger out scores were significantly lower than those in the control group (p<0.001). In mood profile evaluation, although there was no difference in scores of confusion, fatigue, anxiety, depression and anger subscales between both patient groups, all were significantly higher than in the controls (p=0.014, p<0.001, p=0.011, p<0.001 and p=0.003, respectively). Conclusion: These findings suggest that patients with TTH cannot express outward their anger, which is increased due to psychological or physical factors, so that inward anger can be increased in these patients; accordingly, the level of headache can be increased through somatization.
